Core Teams
An AXIS Healthcare Core Team consists of a Health Coordinator (a nurse), a Resource Coordinator (coordinates social services), and a Member Service Coordinator. Your team becomes fully acquainted with you and your health needs by conducting a comprehensive assessment and health history. The coordinators partner with you and your caregivers to insure that you receive the care, services and equipment you need when you need them.
Your coordinators can answer questions and help in many areas including:
- Medical Issues/Concerns
- Coordinating House Moves
- Accessing Legal Resources
- Arranging Appointments and Other Services
- Arranging Transportation
- Completing Paperwork
They can also help you with employment resources and help you keep your eligibility for public programs and/or private insurance.
