About Us

AXIS Healthcare provides revolutionary health care coordination to over 1,500 Minnesotans with disabilities.

AXIS was created in 1997 by Courage Center and Sister Kenny Rehabilitation Institute as a way to help people with disabilities get the care and medical attention they need when they need it. We focus on delivering services to prevent health complications and improve quality of life.

We also work to ensure effective communication between our members, their families and caregivers, and health providers and caregivers.

• AXIS works to find solutions to your Healthcare needs so you can maintain or regain your independence.
• AXIS assigns you a Care Coordinator with access to a team of dedicated professionals including a Health Coordinator, Resource Coordinator and Member Services Coordinator.
• AXIS is a one-stop-shop for your disability care coordination needs.

Our approach to disability care has led to better healthcare outcomes for our members, including shorter hospital and nursing home stays, decreased emergency room visits, and more efficient processing of medical supplies and equipment.

Sister Kenny Rehabilitation Institute and Courage Center are nonprofit organizations with more than 100 years of combined rehabilitation service to people with disabilities.
