Care Coordination

AXIS Healthcare has updated the Care Coordination model for our MnDHO program. This change will enable us to offer the highest, most personal level of disability care coordination with our available staff.

In this updated model, members will have single Care Coordinator to serve as their primary contact for all their health and social service needs. This Care Coordinator is still supported by an interdisciplinary team (that includes a Health Coordinator, Resource Coordinator and Member Services Coordinator), but members will work through one coordinator as the primary contact.

The level of assistance AXIS Healthcare members have come to know and trust will still be there, but this change will allow us to provide every member with the full resources of our entire organization. Members probably won’t notice much of a change in your everyday interaction with AXIS Healthcare, but you may find yourself working with some new AXIS staff. It is our sincere hope that this change enables our work with you to run smoother and allows us to offer the best disability care coordination possible.
