Am I Eligible?
Note: We are no longer accepting applications to join AXIS Healthcare through the MnDHO program, which will be ending at the end of 2010. Please know that we are committed to serving our current MnDHO members through the end of 2010. Current Members of AXIS through the MnDHO program should know:
- While you will need to decide on an alternative to MnDHO, you DO NOT need to take any action at this time. Be sure you consider all your options before making your decision.
- You are still covered under MnDHO for the rest of this year and will continue to receive care coordination support from AXIS Healthcare.
- AXIS will be here. We will continue to operate as a care coordination care program and will be with you through this transition.
- Member information meetings about the transition and the options available are scheduled during August and September. Dates are posted on this site (create link??)
- While AXIS staff cannot direct you with your choices, we can help educate you about your options.
- If you have waiver and/or PCA services your AXIS care coordinator should be involved in communication with the respective counties so that a planned transition can be developed
- If you desire to keep AXIS care coordination, you could choose Medica Accessibility when you decide to transition off MnDHO; AXIS provides care coordination for this program
- AXIS is working on securing other contracts and there may be additional options available later this year that will allow you to keep AXIS care coordination.

Be a person with a certified physical disability;Be eligible for Medical Assistance with or without Medicare;Be at least 18 years old and under age 65 years;Live in the seven-county Twin Cities metro area (Hennepin, Ramsey, Anoka, Dakota, Washington, Carver or Scott counties.)
