When you become an AXIS Healthcare member, you are assigned and have continual access to a Core Team, which includes your Resource Coordinator, Member Services Coordinator and your Health Coordinator.

Health Coordinators are experienced nurses who understand the healthcare system and the unique needs of people with disabilities. 

The first thing you will do as a member of AXIS is meet with your Health Coordinator to develop a unique plan for your independent living. That plan will include the proper amount of ongoing support that is best for you, your family and your caregivers.

Your Health Coordinator will remain your key contact for all ongoing health activities while you are a member of AXIS. Whenever you have a health-related question, you can call your Health Coordinator directly. Health Coordinators help you get the treatment you need early, so you can spend more time enjoying life in your community and less time trying to navigate your way through the many layers of the healthcare system.

Here are just a few of the many advantages of having a Health Coordinator:

• A nurse is available seven days a week, 24 hours a day, so you don’t have to choose between going to an emergency room or waiting until business hours to ask a medical professional a question.
• Your Health Coordinator understands the unique needs of people with disabilities, so you can talk to your Health Coordinator about any issue that impacts your health, such as stress, illness, seating, housing and transportation – anything that is a part of staying healthy, living successfully and living independently.
• Your Health Coordinator will answer your questions, explain your options and give you as much guidance as you need to make the best decisions about your healthcare.
• The final decision about your health and your healthcare is yours. Your Health Coordinator is simply there to help you make the best choices and decisions possible.
