When you become an AXIS Healthcare member, you are assigned and have continual access to a Core Team, which includes your Health Coordinator, Member Services Coordinator and Resource Coordinator.

Resource Coordinators are professionals experienced in finding programs and services for people with disabilities, and in finding solutions for many of the topics that affect your life.

Your Resource Coordinator knows how to find the nuts-and-bolts solutions that can make community living and the social services systems work best for you. Having your own Resource Coordinator is like having a personal assistant to help you in many areas of life, including:
