Your Care Coordinator

Who to call for assistance with you health care and resource needs.

AXIS Care Coordinators have access to a Health Coordinator (a nurse), Resource Coordinator (coordinates social services) and Member Services Coordinator.  Your Care Coordinator becomes fully acquainted with you and your health needs by conducting a comprehensive assessment and health history.  Your Care Coordinator will partner with you and your caregivers to insure that you receive the care, services and equipment you need when you need them,

Your Care Coordinator can answer questions and help in many areas, including:

• Medical Issues/Concerns
• Coordinating House Moves 
• Accessing Legal Resources
• Arranging Appointments and Other Services
• Arranging Transportation
• Completing Paperwork

Care Coordinators can also help you with employment resources and help you keep your eligibility for public programs and/or private insurance.
