AXIS HEALTHCARE COORDINATION SAVES MONEY, IMPROVES LIVESAXIS AFTER HOURS CALL LINE SAVES NEARLY $100,000 A MONTH IN AVOIDING HOSPITALIZATIONS
AXIS staff answers about 45 calls per month from our members after business hours, providing support on evenings and weekends when the office is closed. We divert about 16% of those clients who would have gone to the hospital to other appropriate alternatives by offering a variety of in-home options, including activation of pre-arranged crisis or health care management plans, arranging an in-home visit by a nurse the following day, or by reassuring the client with careful watchful waiting. This program results in diverting about 7.25 hospitalizations per month. Using 2008 claims data, the average cost of a day in the hospital for our members is $2,031/day, and the average length of stay of a Minnesota Disabilities Healthcare Options (MnDHO) member is 6.77 days. With this program, we avoid $99,686/month in hospitalizations.
AXIS MEMBERS EXPERIENCED OVER A 70% INCREASE IN HEALTHY DAYS AFTER ONLY 90 DAYS
Individuals served by AXIS Healthcare have disabilities and complex medical conditions. Initially they report poor health related quality of life, as reflected in our data from the Health Days Survey. For comparison, a national survey of individuals who needed help with routine care reported 12.5 healthy days per month, and those that needed help with personal care reported 9.8 healthy days per month. On enrollment in AXIS, members reported only 6.5 healthy days per month (US DHHS, 2000). After only 90 days of participation in AXIS, our members reported 11.2 healthy days per month, an increase of 4.7 days per month. The benefits of care coordination are clearly demonstrated in the improved health of our members.
AXIS CARE COORDINATION HELPS MEMBERS ACCESS THE CARE THEY NEED
Each member who works with AXIS Healthcare receives an evaluation and an individualized care plan. These care plans include management of acute health care concerns, as well as longer term goals and plans. In addition to having complex health care needs, our members often lack basic preventive health care. Access to specialty and primary care providers is the most common need for our clients, taking just slightly less than a month to schedule and attend an appointment. Access to transportation is the second most common problem, taking about 10 days to facilitate access to transportation. Other common needs are access to medication, fitness facilities, adaptive equipment, nursing or therapy. Meeting these needs provides our clients with the basic health care and social support they need to be successful in participating in the community.
