Making a Difference

AXIS Healthcare’s innovative model of disability care coordination is helping people with disabilities find and receive the care they need when they need it. This refreshing change in disability care is making a huge difference in the lives of our members and our community.

For example, as a result of the individualized attention given by AXIS Care Coordinators, our members have experienced shorter hospital stays, decreased emergency room visits, more appropriate and well-coordinated medical attention and more efficient processing of medical supplies and equipment. That means medical and insurance costs have been reduced and our members can return to being healthy, independent, active members of our community.
