To enroll with AXIS Healthcare through Medica AccessAbility Solution you must be:
Certified disabled by the Social Security Administration or State Medical Review TeamAt least 18 years old, and under the age of 65 at the time of enrollmentEligible for Medical Assistance · Have Medicare Parts A and B or no MedicareLiving in Medica AccessAbility Solution’s service area (Anoka, Carver, Chisago, Dakota, Hennepin, Isanti, Ramsey, Scott, Sherburne, Washington and Wright Counties)
