AXIS Healthcare provides care to Minnesotans living in the seven-county Twin Cities metro area who are affected by disabilities. To become a member of AXIS Healthcare and receive access to the services you need, you must:
Be a person with a certified physical disability;Be eligible for Medical Assistance with or without Medicare;Be at least 18 years old and under age 65 years;Live in the seven-county Twin Cities metro area (Hennepin, Ramsey, Anoka, Dakota, Washington, Carver or Scott counties).
