Who We Serve

AXIS Healthcare provides care coordination to Minnesotans on MnDHO living in the seven-county Twin Cities metro area (Hennepin, Ramsey, Anoka, Dakota, Washington, Carver or Scott counties.)   

We also provide disability care coordination to Minnesotans on Medica's AccessAbility program who are living in the 11 metro area (Hennepin, Ramsey, Anoka, Dakota, Washington, Carver, Scott, Isanti, Sherburne, Chisago or Wright counties.)
