Am I Eligible?

AXIS Healthcare provides care to Minnesotans living in the Twin Cities metro area who are affected by disabilities. You can be eligible to receive healthcare coordination services through AXIS if you enroll in a health plan that contracts with us. Currently we have contracts with two health plans: Medica and UCare.

To become a member of AXIS Healthcare and receive access to the services you need through Medica AccessAbility, you must:
  1. Be a person with a certified disability;
  2. Be eligible for Medical Assistance with or without Medicare;
  3. Be at least 18 years old and under age 65 years;
  4. Live in the eleven-county Twin Cities metro area (Hennepin, Ramsey, Anoka, Dakota, Washington, Carver, Scott, Isanti, Sherburne, Chisago or Wright) counties.
To become a member of AXIS Healthcare and receive access to the services you need through UCare Connect, you must:
  1. Be a person with a certified physical disability;
  2. Be eligible for Medical Assistance with or without Medicare;
  3. Be at least 18 years old and under age 65 years;
  4. Live in the seven-county Twin Cities metro area (Hennepin, Ramsey, Anoka, Dakota, Washington, Carver or Scott) counties.
To receive County Case Management Services through AXIS Healthcare, you must:
  1. Be a person with a certified disability;
  2. Be eligible for, and active within the Medical Assistance Program – with or without Medicare;
  3. Be at least 18 years old and under age 65 years;
  4. Be assessed by the county as being eligible for, and opened to county waiver services under the Community Alternatives for Disabled Individuals (CADI) or the Traumatic Brain Injury (TBI) waiver;
  5. Your County of Financial Responsibility is Hennepin, Ramsey, or another county that establishes a host-county contract with either of those counties.
